What is the effect of APRN-led telehealth versus usual care on readmissions and quality of life among community-dwelling adults with heart failure?
Adult participants 18 years and over diagnosed with heart failure dwelling in the community. Heart failure patients that are pediatric and adult in acute care settings or residing in a care facility will be excluded.
The Advanced Practice Registered Nurse will evaluate data obtained by an automated telehealth program and intervene with patients showing heart failure symptom exacerbation or lapse in self-management or follow-up.
Comparison is usual care defined as no APRN-led telehealth intervention.
Readmission rate among community dwelling adults with heart failure in the intervention group heart failure related quality of life rate among community dwelling adults with heart failure in the intervention group as measured by the valid and reliable instrument Minnesota Living with Heart Failure Questionaire (MLHFQ).
